Post-partum haemorrhage and what causes it?

I had it both when giving birth to my son and daughter through c-section. Not sure what causes it. Almost need to remove the womb but luckily it stabilises eventually so no need to remove womb. But I am still puzzled why I have it and both time I need blood transfusion. I heard it is getting more common nowadays.
